Heart failure according to New York Heart Association (NYHA) functional class is divided into four, namely: 
1. Shortness symptoms in heavy physical activity 
2. Shortness of symptoms in moderate activity 
3. Shortness of symptoms in mild activity 
4. Symptoms of shortness of very mild or activity break.

Clinical manifestations
Clinical manifestations of heart failure as a whole is highly dependent on etiology. However, can be described as follows:

- Ortopnue
- Dyspnue on effort (DOE) that is congested when doing activities.
- Nocturnal paroxysmal Dyspnue (PND) is a sudden shortness of breath
disretai cough at night
- Pounding
- Hurry tired
- Cough-cough.

Diagnosis of Heart Failure
Diagnosis of heart failure is established based on primary and / or additional criteria.
The main criteria
-Ortopnue
-Nocturnal paroxysmal Dyspnue
-Kardiomegali
-Gallop
-Increased JVP (jugular Venus Pressure)
-Reflex hepatojugular

Additional Criteria
- Ankle edema
-Coughing at night
-Dyspnue effort on
-Hepatomegaly
-Pleural effusion
-Takhikardi.

Diagnosis is established on the basis of the existence of two major criteria or one major criterion with two additional criteria.
